Minutes of December 19, 2005

 

 

The Idaho State Cosmetology Board meeting was called to order at 1:35 p.m., Monday, December 19, 2005 at the Bureau of Occupational Licenses, Owyhee Plaza, 1109 Main Street, Suite 220, Boise, Idaho.

 

Roll Call:                      Eric Negaard, Chair

Linda Swope, Board Member

Mary Lambert, Board Member

                                    Budd Hetrick, Deputy Bureau Chief

                                    Marcie McGinty, Investigations Coordinator    

                                    Stephanie Guyon, Deputy Attorney General

 

Joan (Joni) Huff and Geneal Thompson, Board Members, absent, excused.

 

FORMAL DISCIPLINARY ACTION

 

Stephanie Guyon, Deputy Attorney General submitted a Memorandum to the Board regarding formal disciplinary case numbers:

 

COS-2006-2

COS-2006-3 and COS-2006-4

COS-2006-6 and COS-2006-7

COS-2006-8 and COS-2006-9

 

After review and discussion, the Board made their recommendations to Ms. Guyon in conjunction with the recommendations in the memorandum for disciplinary action.  No motions were made.

 

Ms. Guyon presented a signed Stipulation And Consent Order in the matter of Travstar L.L.C dba Scot Lewis Schools, License No. CC-226474.  After full discussion and review, it was moved by Linda Swope to accept the signed Stipulation And Consent Order as written.  Seconded by Mary Lambert, motion carried.

 

INVESTIGATIVE REPORT

 

Budd Hetrick submitted an investigative report to the Board.  After full review and discussion the Board made the following motions.

 

COS-L1A-1-2004-12:  Moved by Mary Lambert to accept the recommendations of the Bureau for closure.  Seconded by Linda Swope, motion carried.

 

COS-B1-1-2005-58:  Moved by Linda Swope to accept the recommendations of the Bureau for closure.  Seconded by Mary Lambert, motion carried.

 

COS-B1-4-2006-12 thru 15:  Moved by Mary Lambert to accept the recommendations of the Bureau for closure for case numbers COS-B1-4-2006-12, 13, 14, & 15.  Seconded by Linda Swope, motion carried. 

 

COS-L6-3-2006-19:  Moved by Linda Swope to accept the recommendations of the Bureau for closure.  Seconded by Mary Lambert, motion carried.

 

COS-B1-4-2006-36:  Moved by Mary Lambert to accept the recommendations of the Bureau for closure.  Seconded by Linda Swope, motion carried.
